Public Records Request Form

City Clerk Signature Logo 2025

The City Clerk is the custodian of public records for the City of Belle Isle.  Any requests for records from another department will be coordinated through the City Clerk's office.  When making a public records request, there is no requirement to provide a name or reason for the request.  Additionally, requests do not need to be submitted in writing.  To provide the best response, consider the information that will be useful.  Broad public records requests take longer and may be costly for the requestor.  You may use the form below, visit City Hall, or call 407-851-7730.

Please read all of the information below before submitting a request, and be as specific as possible when requesting records.  Providing contact information is optional. If you prefer not to provide it, please call City Hall every two business days to provide any necessary clarifying information and to monitor the status of your request.

Note: Florida law does not require you to provide your name or complete this form. We request this information so that we can track the status of your request and ensure it is met promptly. Often, your request must be routed to a particular office where the information is maintained, or we might need to contact you to clarify your request. Our goal is to fulfill your request as expeditiously as possible.

  • Please include the date the document was created or a date range (required).
  • For electronic records, keywords and search terms that are overly broad may result in responses that contain thousands of documents.
  • If you are requesting records from the Belle Isle Police Department, please provide an incident number, if available.

While City policy requires that we charge costs if extensive labor is involved in locating or duplicating your records, with specific information, we can prevent or minimize any such labor costs.  City policy requires a fee if extensive labor is needed to locate or duplicate your records. If we estimate that there will be a cost associated with your request, we will let you know before we work on your request. If the cost is less than $100, we will work on your request as soon as you ask us to. If the estimated cost is greater than $100, we need payment in full before we can work on your request.

  • Charge for Copies: 8 1/2 x 14 or smaller: $0.15/page, Double-sided: $0.20/page, Color: $0.25/page. In addition, there is a service charge for any reasonable time spent over and above 15 minutes to fulfill a records request (the first 15 minutes shall be at no charge). This charge is determined based on the labor cost of the personnel providing the service. A cost estimate may be provided before the request is filled.

Receive your records: You will receive an email notification when your records are available. You can receive your documents electronically, by fax, or by mail. You can also pick up your records in person.
